The Management Team, consisting of Geriatric Clinical Nurse Specialist and/or a Geriatric Social Worker, will visit the older adult in their home and assist in resolving concerns about physical, emotional, social or daily living requirements. Working closely with the older adult family, their physician and other care providers, assistance will be provided in locating and arranging services in the community that will meet the older adult’s needs.
