The Michigan Department of Health and Human Services (MDHHS) Bureau of Aging, Community
Living, and Supports would like to hear from you! We would like to learn about your community
and what you feel you need as you get older to help you maintain your independence. Your
views are important, and we would really appreciate your participation in this survey. All your
responses will be kept private and will be used to inform the State Plan on Aging developed by
the Bureau. This plan will go into effect beginning in October of 2023. If you have questions
